It depends on the head coach. Just to look at 2 Super Bowl winning coaches from recent history:
Mike McCarthy runs the Packers offense. He oversees the final gameplan each week, runs the offense in practice, calls the plays, etc.
John Harbaugh, on the other hand, gives his coordinators a lot more autonomy, similar to what Joe Philbin did here. He 'makes suggestions,' but he lets his coordinators run their units all week and call the plays on gameday.
