So, Eddie Johnson and John O'Brien not starting for a second consecutive time probably means Arena intended them to be supersubs from the outset? It also means O'Brien isn't fit to go 90 minutes and hasn't recovered completely.

That's correct, but it is more a testament to Dempsey than anything else. Dempsey is a genius at getting fouls, even if he does nothing else.
